Smrity Bagga
Director & Owner
Smrity is a registered Occupational Therapist and for more than 20 years, she has worked across various positions within Community Health, Sub-Acute Care and Rehabilitation. She has a Master’s in Health Administration and for the past 10 years, has lead a team of highly skilled Allied Health Professionals who have provided care and planned successful discharge for patients requiring assistance following an acute medical illness, trauma or general age related decline.
Smrity is passionate about promoting independence and supporting people within their own environment. In 2016 Smrity developed and successfully rolled out Home Rehab for a major private Rehabilitation facility. This program helps patients to be discharged home directly from acute hospital care following hip or knee surgery and is supported by Allied Health teams within their own environment.
With the same passion and energy, she embarked on bringing Home Care Assistance to South East Melbourne, aiming to “change the way our community ages” because living at home is the best way to Live!
Sally
Clinical Care Manager
With a strong background in healthcare and a passion for providing exceptional person-centred care, Sally works in conjunction with the clinical team to deliver outstanding clinical assessments and care coordination to clients. She collaborates on clinical care outcomes, ensuring that clients receive the best possible care tailored to their specific needs.
Sally is also a Registered Nurse and plays a vital role in supporting the team and overseeing the clinical care requirements of both our team members and clients alike.
Sue
Employee Care Manager
Coming from an extensive human resources background, Sue is primarily responsible for screening and recruiting support workers in line with the organisation’s values and ethics. Sue also provides ongoing support and feedback to our support workers to ensure they have the most updated skills and knowledge to perform their jobs to the highest standards. She also ensures we operate within government regulations to ensure quality compliance. This important process offers peace of mind and confidence to our clients and their families.

Khushi is a Registered Nurse and passionate about enhancing and making a positive difference to our client’s quality of life. Currently she is on leave enjoying family time.

Jessica has a passion for learning and understanding the life stories of individuals, from where they have come from to where they are presently. Her goal is to create relationships with each client and encourage them through the changes at hand.
Out of work, Jessica appreciates great food and spending time with her loved ones. She enjoys exploring new towns, hiking and being involved in her local church.

Caitlyn is a dedicated nurse with a wealth of experience in aged care, spanning across community care, residential aged care, and geriatric hospital wards. Transitioning to the role of HCP Care Manager, she is deeply committed to supporting seniors in maintaining a safe and comfortable lifestyle in their own homes.
Drawing from her extensive healthcare and community care background, Caitlyn possesses a strong grasp of personalized care requirements for her clients. She implements tailored strategies aimed at improving their quality of life and fostering independence.

Nikki is the heart of our NDIS Care Management team, driven by her passion for supporting others. With a background in Psychology and Counselling, her journey through various roles in human services and resources has equipped her with a diverse skill set.
Nikki thrives on establishing meaningful connections with her clients, prioritizing personalized care plans that cater to their individual needs. A staunch advocate for those under her care, she is dedicated to ensuring that each client receives the support and attention they deserve.
